AMRcourse Payments and Completion Certificates

For AMRcourse course-fee payments, we associate the selected course, amount, payment and refund status, and transaction identifiers with your Moodle account. We use these records for payment verification, course administration, support, and accounting. Card details are sent directly to Square through its dedicated payment form and SDK. Our servers do not receive or store card numbers or security codes. PayPay payments are processed in PayPay's app or payment page.

Square's payment SDK may process user and device identifiers, product interactions, and diagnostic information such as crash data for payment functionality, security, diagnostics, and service improvement. Square's and PayPay's own privacy policies also apply to their processing.

If you request verification of a HeartCode BLS completion certificate, we process your name, certificate number, issue date, and submitted PDF to verify eligibility. Access is limited to you and authorized administrators. Certificate information and files are deleted 90 days after the course end date. Payment and accounting records are retained separately for operational and legally required purposes.

AMRcourse Account and Data Deletion

To request deletion, open the app menu, choose Delete account, and select Delete app + MOODLE. You can also email info@amrteamtokai.com from your registered email address with the subject AMRcourse account deletion. You may request deletion of specific personal data without deleting your account through the same contact.

After verifying and processing your request, an administrator deletes the account and associated learning data. Payment and accounting records are retained separately for operational and legally required purposes. Completion certificate information and files are deleted 90 days after the course end date, as described above. We will explain any records that must be retained and the applicable retention period when responding to your request.

合同会社AMR Team TOKAI（以下「当社」といいます。）は、当社が提供するウェブサイト、学習システム（Moodle等）、およびモバイルアプリケーション（以下「本サービス」といいます。）における、ユーザーの個人情報の取扱いについて、以下のとおりプライバシーポリシー（以下「本ポリシー」といいます。）を定めます。

第1条（個人情報）

「個人情報」とは、個人情報保護法にいう「個人情報」を指し、生存する個人に関する情報であって、氏名、所属、メールアドレス、電話番号、位置情報、端末情報、学習履歴その他の記述等により特定の個人を識別できる情報を指します。

第2条（個人情報の収集方法）

当社は、ユーザーが本サービスを利用する際に、以下の情報を取得することがあります。

  • 氏名、所属機関、メールアドレス等の登録情報

  • 学習履歴、成績、受講状況等の教育関連データ

  • お問い合わせ内容

  • アクセスログ、IPアドレス、Cookie情報

  • 端末識別情報、OS情報等の技術情報

また、モバイルアプリではアプリの機能提供のため、一部の情報がユーザー端末内に保存される場合があります。

当社が個人情報を収集・利用する目的は、以下のとおりです。

第4条（モバイルアプリに関する情報の取扱い）

当社は、提供するモバイルアプリにおいて、アプリの機能提供および改善のため、端末情報、利用状況、クラッシュ情報等を取得する場合があります。

第5条（Cookieおよび類似技術）

本サービスでは、利便性向上および利用状況の分析のため、Cookieおよび類似技術を使用することがあります。ユーザーはブラウザの設定によりCookieを無効にすることが可能です。

第6条（第三者提供）

当社は、法令に基づく場合を除き、本人の同意を得ることなく第三者に個人情報を提供することはありません。

第7条（外部サービスの利用）

本サービスでは、サービス向上のために外部サービス（アクセス解析、クラウドサービス等）を利用することがあります。

第8条（個人情報の安全管理）

当社は、個人情報の漏えい、滅失または毀損の防止その他の安全管理のため、必要かつ適切な措置を講じます。

第9条（開示・訂正・削除）

ユーザーは、当社の保有する自己の個人情報について、開示、訂正、追加または削除を請求することができます。

第10条（未成年の利用）

未成年のユーザーが本サービスを利用する場合、必要に応じて保護者の同意を得るものとします。

第11条（本ポリシーの変更）

本ポリシーの内容は、法令その他本ポリシーに別段の定めのある事項を除いて、ユーザーに通知することなく変更することがあります。

AMRcourseの決済・修了証情報の取扱い

AMRcourseの受講料決済では、受講商品、金額、支払い・返金状況、取引識別子をMoodleの利用者アカウントと関連付け、入金確認、受講手続き、お問い合わせ対応、会計記録のために取り扱います。カード情報はSquareの専用画面・SDKでSquareへ直接送信され、当社サーバーではカード番号・セキュリティコードを取得・保存しません。PayPay決済はPayPayのアプリ・決済画面で処理されます。

Squareの決済SDKは、決済処理、安全性の確保、障害診断・サービス改善のため、利用者・端末の識別情報、操作情報、クラッシュ等の診断情報を取り扱う場合があります。SquareおよびPayPayによる情報の取扱いには、それぞれのプライバシーポリシーも適用されます。

HeartCode BLS修了証の確認を申請する場合は、氏名、修了証番号、発行日および提出されたPDFを、修了資格の確認のために取り扱います。閲覧は本人および権限を持つ管理者に限定し、コース終了日の90日後に削除します。決済・会計記録は修了証とは別に、業務および法令上必要な期間保存します。

AMRcourseのアカウント・データ削除

アプリのメニューから「アカウント削除」→「アプリ＋MOODLE」を選択して申請できます。または、登録済みメールアドレスから info@amrteamtokai.com へ、件名を「AMRcourseアカウント削除」としてご依頼ください。アカウントを残したまま個別の個人情報の削除を希望する場合も、同じ窓口へご相談ください。

管理者が申請を確認・処理した後、アカウントと関連する学習データを削除します。決済・会計記録は、業務および法令上必要な期間、別途保持します。修了証情報とファイルは、前述のとおりコース終了日の90日後に削除します。保持が必要な記録と適用される保存期間は、ご依頼への回答時にご案内します。
